I would sign a card to my nephew as 'Aunt Pam'.

How do I sign a card to his girlfriends child?

Hi Pam,

Thank you for the question. How does the child refer to you? If he/she refers to you as "Aunt Pam", then sign it Aunt Pam. If he/she refers to you as "Mrs. Scheaffer". If the child is too young to speak, then sign it based on how the girlfriend addresses you.
